Cherokee Nation Public Health will be at Welch Public Schools on Monday October 5, 2026, from 10:30 am to 12:30 pm to provide Flu vaccines at no cost to participants. Consent forms MUST be completed and signed for CNPH to provide the vaccination.
If the participant is a minor, a parent or guardian MUST designate on the CNPH registration and consent form if the minor can be vaccinated with or without your presence.
Consent forms will be available in your school office.
